Enrollment Procedures
Parents/Students must complete the following procedures in order to be enrolled at Peniel Baptist Academy. Students are not eligible for school attendance until ALL these items have been completed: 1. All fees must be paid according to the annual Tuition and Fee Schedule (see the Financial Information"). 2. All financial charges incurred from the previous year must be paid current prior to registration and prior to the start of the school year. 3. Parents/Students must complete all registration materials including: a. The registration form signed and dated by parents. b. The medical and emergency information form completed by parents. c. The medical release form signed by parents and notarized by a public notary. d. The field trip permission form signed and dated by parents. e. The library policies form signed and dated by parents. (K-12th) f. The internet and computer use policy and consent waiver form signed and dated by parents and students. (4th-12th) g. The permission to paddle form signed and dated by parents. (K-12th) 4. *IMPORTANT: Parents (PK2-12th) and Students (4th-12th) must sign and date the Statement of Agreement signifying agreement with all PBA policies. This form is found in the back of the Student Handbook. 5. Parents must provide the records department with the following critical items: a. Birth Certificate (copy) b. Social Security Card (copy) c. Shot record (Florida Blue Card with current immunizations)
d. School Entry Health Exam (canary copy DH 3040)
6. Parents of students transferring from another school must provide PBA with the previous school's address so that the student's cumulative records may be transferred. 7. Students entering K-12th grades must pass an entrance examination(s). The School Administration reserves the right to recommend a lower grade placement or not admit a student based upon their entrance examination results in relationship with their previous academic performance. 8. Parents and students must attend and participate in a personnel interview with the School Administration. Previous records detailing the student's past school performance will be required during this interview, e.g. report cards. Students re-enrolling must complete steps 1, 2, 3 and 4 of the "Enrollment Procedures". PBA reserves the right to refuse re-enrollment to students or students of families who have demonstrated disharmony regarding the purpose, objectives, standards, policies, rules and regulations of Peniel Baptist Academy.
Credits and grades earned and offered for acceptance shall be based on official transcripts and shall be accepted at face value if earned through traditional sources that are accredited by a recognized regional or state accrediting agency without further validation. Traditional sources are defined to be a public and nonpublic comprehensive secondary and middle schools and vocational-technical institutions.
